Canberra Hip & Knee Replacement- Calvary Clinic is a private orthopaedic practice which
offers a wide range of treatments for private patients with Hip and Knee disorders.

As a Specialist clinic, patients need referral from their General Practitioner,
or other doctor to be eligible for a rebate from Medicare.

When you come for your appointment Remember to bring the following:

Veteran affairs card.
Referral letter from your GP, family physician or other doctor.
Have your insurance information.
Copies of operation records, medical records, x-rays, MRIs, CT scans relevant to your condition.
If you have seen a physiotherapist, please bring a progress letter from the therapist.
If you have had surgery elsewhere, please bring a copy of your operation report.
List of current medications and past medical conditions.

Very often during the course of your evaluation or treatment for an orthopaedic condition your
doctor will require x-rays to determine the cause of a problem or to evaluate your progress. If not
all the relevant information is available at the time of your appointment this may cause a delay in
your treatment. In addition it may be necessary to repeat a study to complete your evaluation.
